I give my permission for my child/children to participate in all Sports Camp activities and I release Tahlequah First Baptist Church staff and volunteers from liability in case of injury or harm that may occur during or due to my child’s participation in this event. I authorize the Sports Camp staff to provide my child emergency medical care and to transport him/her to and from the hospital. I agree to pay for all charges that are a result of such treatment

I give my consent for Tahlequah First Baptist staff or persons who have been given permission by Tahlequah First Baptist staff to use photos, videos and/or audiotape that feature or include my child and to use work done by my child as well as my child’s first name for the purpose of publicizing and promoting Tahlequah First Baptist Church ministries. I understand that these images, text, or recordings may be published in newspapers, magazines, or on the World Wide Web, may be broadcast on television or radio, and/or may be presented in Tahlequah First Baptist informational or promotional materials or presentations.

If your child has COMPLETED Kindergarten, they will be participating in Team K. Team K is specifically designed with our youngest campers in mind. Each day a sport will be highlighted and basic skills for that sport will be taught.

COMPLETED 1st - 6th Grade may choose ONE sport from the following options: (Teams will be divided into 1st - 3rd grade & 4th - 6th grade groups) Completed Kindergarten must select Team K.

Because of the size of our program and field space, we will be transporting our Football, Soccer, and Baseball teams to Tahlequah Sports Complex. I give permission to transport my child to and from these activities during Sports Camp.
